Planning a funeral in the advance of need is one of the most thoughtful things a person can do. It simply makes it easier for those you love. Hood Mortuary offers complete funeral pre-planning. One can get his or her wishes down on paper, and, or, pre-fund the services we will provide.

There are a number of advantages to preplanning funeral and cremation arrangements at Hood Mortuary. Here are just a few:

• Your loved ones are not faced with difficult decisions and funeral costs at a time when they really don't want to be.

• The expenses can be guaranteed to cover the funeral costs.

• The funds are placed in a policy that you have full access to at all times.

• Payments can be made for up to 10 years.

• The policy is fully transferable.

 If you do not wish to plan your arrangements over the internet, you may call or visit Hood Mortuary, or you may download these printable forms.  All forms are in Adobe Acrobat (PDF) format.  Upon the completion of the forms, please keep a copy for your records and deliver the originals to Hood Mortuary.  This will ensure the forms will be safe and accessible.

General Price List (with the Casket Price List attached)

This is the itemized listing of prices for all services at Hood Mortuary.

Pre Arrangement Form

This form allows one to record all necessary information including death certificate information, funeral wishes, and obituary information.

Cremation Authorization

The required form that gives the closet next-of-kin's permission for the cremation to be performed.

Declaration of Disposition of Last Remains

This form allows for an individual to specify his/her wishes for disposition, ceremonial arrangements, organ donation, and more (in the state of Colorado only).  It is considered a codicil to a will and his form must be notarized.
